It is important to arrive at least 20 minutes before your first visit to ensure there is plenty of time to sign and complete your paperwork. If you are already an existing patient of Wallace Foot & Ankle Center, please arrive at least 5 minutes before follow up visits.

Upon arriving at your first appointment, please ensure you have the following items:

1. Driver’s license (photo ID)
2. Insurance Cards
3. Completed New Patient Information Form (Download Here)
4. Completed Patient History Form (Download Here)
5. Co-pay/Payment
